作业人员安全承诺书（中英文）

OPERATOR’S SAFETY COMMITMENT

针对中世运坦桑尼亚管道天然气项目特点，为规范作业人员安全行为，防止和杜绝“三违”现象，保障作业人员生命安全和堆场生产作业的顺利进行，特制定本承诺书。

This is to stipulate the COMMITMENT hereinafter as per the characteristics of Sino Worlink Tanzania Natural Gas Pipeline Project, hence to specify operators’ conduct safety in case of “three violation” and guarantee safety of operators’ life and property as well as smooth production at storage station.

1. 所有新进现场作业人员都必须认真学习本承诺书。

All the new mobilized operators shall seriously go over the COMMITMENT hereby.

2. 劳动纪律

Labor Discipline

1. 未接受安全教育的作业人员严禁进入生产区域。

No access to working areas for those operators without safety training.

2. 进入作业现场人员必须按规定佩带劳防用品。

Any site personnel to enter the working spot shall wear protection equipment as required.

3. 作业前，要对参与作业的设备、工具、吊索等按规定进行安全检查，确保合格后方可使用作业。

No use of the equipments, tools, hooks or slings for work purpose unless the stipulated pre-work safety inspection is already conducted to be accepted as qualified.

4. 作业人员作业过程中做到不伤害自己，不伤害他人，不被他人伤害。

No hurt or injury to himself or others or hurt by others during work procession.

5. 进入作业现场后，不得串岗，嬉戏打闹。

No shifting post or horseplay once access to working spot.

6. 重要操作岗位人员在岗期间严禁接、打手机。

No use of cell phone particularly for those in critical posts.

7. 未进允许不得进入危险区域，不得穿越吊装区域，严禁在吊车工作半径内逗留或作业。

Without permission, no entrance to dangerous areas, no cross the hoisting section, no linger or work within the operating radius of the crane.

8. 任何人员不得在作业现场随意丢弃垃圾等废弃物，作业完毕后要将废弃物清理到指定的地点，做到工完料尽、场地清，确保作业现场的清洁。

No littering about at working spot; in contrast, the waste shall be disposed into the given places to work out all materials and clear the site for the purpose of clean operating spot.

9. 不准乱动各种开关、设备、设施及驾驶场内机动车辆。

No mess around various switches, equipments, facilities and any motor vehicle inside the storage station.

10. 严禁特种作业无有效操作证人员上岗操作；

No work without valid certificate for special operation.

11. 严禁违反操作规程操作；

No operation beyond specification or procedures.

12. 严禁无票证从事危险作业；

No undertaking dangerous operations without certificate.

13. 严禁脱岗、睡岗和酒后上岗；

No dereliction, sleep at work and drunk work.

14. 严禁违反规定运输民爆物品、放射源和危险化学品；

No transport those civil explosives, radiation sources and dangerous chemicals against relative regulations.

15. 严禁违章指挥，强令他人违章作业。

No command against rules or forcing others to work against rules

16. 进入作业现场人员要听从现场HSE监督的管理，HSE监督员有权对各类违章行为进行制止、处罚。

Those site operators already access to the spot shall follow HSE personnel orders, who gets the authority solely to hold down or punish any conducts against operation regulations and rules.

3. 劳动保护

Labor Protection

1. 作业人员都应按照规定配备安全帽、工作服、防砸鞋、手套等必要的劳防用品。安全帽应符合相关标准。工作服应轻便、透气、舒适，适合工作环境和自然环境。防砸鞋必须穿皮制的、超过脚踝的、带有脚趾保护的钢头工靴。

All the operators shall be, as per stipulations, equipped with safety helmets, overalls, safety shoes, gloves and other necessary protection equipments. And safety helmet shall comply with relevant standards.; overalls shall be lightweight, breathable, comfortable, suitable for working surroundings and natural environment; safety shoes shall be steel-headed work boots made of leather at least over ankles to protect toes.

2. 凡从事打磨、切割、电焊等对眼睛有伤害的作业，作业人员应当配备适合于该工作的护目镜、面罩或其他防护用品。

Those who undertake such operation harmful to eyes as grinding, cutting, welding shall be equipped with suitable goggles, masks or other protection articles.

3. 所有暴露于噪音水平超过85分贝或指定水平的环境中的人员应配备适当的听力防护设备。

Those operators exposed to the environment over 85 decibels or stipulated levels shall be equipped with appropriate audio-protective devices.

4. 员工进入现场作业区不能佩戴妨碍劳动作业的装饰品。

No wearing the decoration articles hampering from operation after access to site working areas.

5. 劳动防护用品、用具应根据发放标准和使用年限，定期更换。

Those labor protection articles or appliances shall be renewed as per distribution standards and durable years.

4. 劳动权利和义务

Labor Right and Obligation

1. 作业人员在安全措施不到位的情况下，有权拒绝危险作业。

Operators are entitled to reject the dangerous work out of safety protection.

2. 紧急情况下，在采取必要措施后，可以先行撤离危险区域。

Operators may evacuate in advance after necessary measures are already taken in emergency.

3. 作业人员必须遵守现场的各项规章制度。

Operators shall observe all the regulations and rules at site.

4. 出现事故、紧急事件或发现事故隐患应及时向现场HSE管理人员或其他管理人员报告。

Any accident, emergency or potential accident shall be timely reported to site HSE management personnel or other management personnel.

 

 

作业人员承诺：

Operator’s Commitment,

我已认真学习并熟悉上述安全规范，并承诺在实际作业中认真遵守和履行。如果违反上述规定，愿意接受相应处罚并承担后果。

This is to certify that after carefully studying and familiarizing the above safety regulations, I promise to discreetly observe all the regulations to fulfill my duties and I am willing to accept relevant punishment as well as bear the consequences if in breach of the regulations hereinabove.
